We will provide long term care facility assessments, we follow patients in nursing homes, assisted living facilities, retirement communities, the community at large. We attend client care conferences and advocate for our clients. We consult with your care providers to help develop a comprehensive care plan.  We continue to monitor the client in the nursing home setting to make certain the care plan is being followed and their needs are being met.
